The oncology playbook I wish I had during Fellowship

Every fellow remembers the feeling of seeing an unfamiliar disease for the first time.
The guidelines were comprehensive. The papers were essential.
But neither explained how experienced oncologists actually approached the patient. Living Algorithms are designed to bridge that gap by making expert clinical reasoning available at the point of care.
Every fellow has the same experience
Your attending asks you to see a new consult.
It's metastatic renal cell carcinoma.
Or cholangiocarcinoma.

But one question remains: "How would my attending actually manage this patient?"
That's the question many fellows are really trying to answer.
Fellowship teaches more than medicine
The formal curriculum is important. You learn:
Disease biology
Clinical trials
Drug mechanisms
Guidelines
Toxicity management
But some of the most valuable lessons aren't found in textbooks. You learn them by seeing how experienced physicians think.
Why one regimen is preferred over another
Why a patient isn't the right candidate for surgery
Why an expert worries about one laboratory value but not another
Why two reasonable options aren't actually equivalent
That kind of reasoning is difficult to teach and is usually learned one patient at a time.
The hidden curriculum of oncology
Every attending has a mental playbook. It includes questions like:
What diagnosis fits this presentation?
Which details actually change management?
What should I rule out first?
What complications should I anticipate?
What mistakes do trainees commonly make?
These aren't separate from evidence-based medicine. They're how experienced physicians apply evidence to real patients.
Unfortunately, that playbook often exists only in the attending's head.
Guidelines tell you what is acceptable
Experienced physicians explain what they would actually do, which is an important distinction.
While guidelines present every reasonable option, experts help you understand:
Which option they prefer
Why they prefer it
Which patients are exceptions
Where the evidence is strong
Where clinical judgment becomes more important
Those explanations accelerate learning in ways that guidelines alone can't address.
Learning from experts shouldn't depend on geography
Some fellows train at large academic cancer centers, while others train in smaller programs. Some spend every day with nationally recognized subspecialists, while others have fewer opportunities to observe different approaches.
But medical education shouldn't depend entirely on where someone trains. The practical wisdom developed by experienced oncologists deserves to be shared broadly.
A playbook for real-world medicine
Imagine opening a treatment algorithm and seeing not only the recommended pathway, but also:
Why this approach is preferred
The landmark trial supporting the recommendation
Common pitfalls
Important patient selection criteria
Practical monitoring recommendations
Clinical pearls from physicians who treat the disease every day
That's more than a guideline, it's a playbook.
Confidence comes from pattern recognition
One of the biggest transitions during fellowship is realizing that experienced oncologists don't memorize every paper. Instead:
They recognize patterns
They know which questions matter
They quickly eliminate options that don't fit the patient
They focus on the details that change management
Those patterns are difficult to learn from static documents, but become much easier to understand when experts make their reasoning explicit.
Preparing for independent practice
Every fellow eventually reaches the same milestone where the attending steps out, and all of the decisions become yours. This is when practical clinical reasoning becomes just as valuable as medical knowledge.
A good playbook doesn't replace independent thinking, it helps build it.
Learning doesn't end after fellowship
The need for practical guidance doesn't disappear after graduation. Community oncologists routinely care for patients across many different tumor types.
Even experienced physicians encounter diseases they haven't managed recently. Merkel cell carcinoma is just one example.
The same playbook that helps fellows learn can help practicing oncologists quickly refresh their knowledge before seeing a patient. Medicine is a lifelong apprenticeship and the best clinicians never stop learning from one another.
Bottom line
The most valuable lessons in fellowship often come from watching experienced physicians explain not just what they recommend, but why.
